Whereas Mr. …………. S/O ……… while working as …………, in the office…………, reported as died through his wife Mst. ………………., (CNIC No. …………. on ……………… and provided Death Certificate issued by U.C. No. …….Tehsil……… District………
AND Whereas, the widow of late Mr. ……………..also provided the succession certificate issued by the competent court, and requested the as per Government of the Punjab Finance Department Notification No. FD.SR.1/3-10/2004 Dated 15th August, 2017, and further clarified vide U.O. No.FD.SR-1/3/2004(P) Dated 12/09/2017 and even number dated 03/10/2017, her family be given all the financial assistance as admissible.
Therefore, keeping in view the request of the widow Mst. ……….., Death Certificate & Succession Certificate provided, and Gov. of Punjab Finance Department Notification/clarifications quoted above, the following financial assistance is allowed.
1) Lump-sum Grant to the tune of Rs. , as deceased employ was drawing his pay in BS-
2) Deceased family allowed salary along with annual increases in the pay, to which deceased official was drawing till the date of superannuation, i.e., up to (Exact Date of Superannuation against the post of OSD created for this purpose).
3) The deceased family is allowed the retention of Government Accommodation (If allotted to the office in his life) till the age of Superannuation, as per provisions of CSR (Pb), Vol-I (Part-I) read with the Allotment Policy issued by S&GAD on dated 19/1/2009.
4) The deceased family is entitled for appointment of one child including BS-1 to BS-11 on contract or regular basis as per provision of Rules 17-A.
5) Apart from this, the deceased family is also entitled for Other Grants as detailed below:
- i) Marriage, Educational Scholarships, Monthly Benevolent Grant,
- ii) Farewell Grant at the time of Death,
iii) Four Months Salary at the time of Death,
- iv) 365-Days in lieu of LPR at the time of death,
- v) Graduity at the time of Death,
- vi) No deduction on account of Group Insurance,
vii) G.P. Fund Deduction at the option of deceased family.
6) Mr. ………. is nominated as Counsel to assist the deceased family in finalization of provision of entire package within the shortest time.
